Required Information for Booking

  • Last name and First name

  • Gender

  • Date of birth

  • Address – Phone contact(s)

For services provided through the National Health Service:

  • Referral from the General Practitioner (GP) or the Pediatrician of Free Choice (P.L.S.); for phone bookings, the operator will request the last four digits of the referral code.

Note: For services provided through the National Health Service, the referral form must be completed by the prescribing physician and must include:

  • personal information (name, surname, address, tax code);
  • priority class (U, B, D, or P); if not indicated, the request will be considered as priority P = schedulable (D.G. Regione Lombardia no. X/3993 of 04/08/2015);
  • valid exemption code (for eligible patients);
  •  diagnostic query or reason for the request

Documents required at the time of acceptance:

  • Referral from the General Practitioner (GP) or the Pediatrician of Free Choice (P.L.S.)

  • Regional/National Health Card – Tax Code

  • Valid exemption certificate (for eligible patients)

  • Previous clinical documentation
